Our in-network partnership with Blue Cross Blue Shield provides most non-grandfathered employer plans access to our services at no additional cost to employees, including the waiver of all copays and deductibles. We can provide customized lifestyle interventions for patients in the comprehensive treatment and management of diseases such as prediabetes, diabetes, prehypertension, hypertension, high cholesterol, and heart disease.

Our comprehensive Clinical Outcomes Division will provide health risk assessments and detailed, HIPAA-compliant reporting on improvements in employees’ A1c levels, cholesterol, blood pressure and other biometric markers.  Health risk assessments are no longer a unique offering, but our evidence-based LifeZone Assessment is an added feature that can provide a more comprehensive overview of employee lifestyle habits, identifying strengths and weaknesses within the work environment that can contribute to an unhealthy lifestyle.
